Bring your Immunization Card (pink card) with you and if you have Medicaid or Private Insurance bring that card too.

"NOTICE"

PLEASE BE AWARE OF WHAT YOU NEED TO KNOW IF YOU HAVE PRIVATE INSURANCE:

Do you have insurance?

If the answer is No, Medicaid, Healthwave, FirstGuard or Ks Farmworker then you quality for VFC.

If the answer is Yes answer the next question:

Does your insurance cover immunizations?

if “I don’t know” is your answer – YOU must find out!

NO – then you qualify for VFC.

Yes – then we can provide you with private vaccine for your child or children, however you are responsible for what your insurance does not cover.

Why is immunization important?

bulletBecause it will protect your child against diseases that can hurt, cripple or kill!
bulletHere is some important information: What Parents Should Know

Before vaccines, parents in the United States could expect that every year:

bulletPolio would paralyze 10,000 children.
bulletRubella (German measles) would cause birth defects and mental retardation in as many as 20,000 newborns.
bulletMeasles would infect about 4 million children, killing 3,000.
bulletDiphtheria would be one of the most common causes of death in school-aged children.
bulletA bacterium called Haemophilus influenzae type b (Hib) would cause meningitis in 15,000 children, leaving many with permanent brain damage.
bulletPertussis (whooping cough) would kill 8,000 children, most of whom were under the age of one.
